Student Registration

Parents and/or guardians may download the forms below as appropriate to enroll their child in a Warren Township School. All forms are PDF and require Adobe Reader. Click on the icon to get your free Adobe Acrobat PDF reader.

After downloading and filling out the appropriate forms, please call the main office of the school(s) that your child(ren) will attend to make an appointment to complete the registration process. All three forms (Registration, Residency, and Health) must be completed and submitted in order to complete the registration process.

Please bring the following documents with you to that appointment to facilitate the enrollment process:

IMMUNIZATION RECORDS PLUS PHYSICAL (from the state of NJ within the last year) PRIOR TO ENTERING.

Written Proof of Residency (e.g., Deed or Mortgage, Lease Agreement, Recent Utility Bill, or Driver’s License)

Scholastic Records from Previous Schools (e.g. Student’s Latest Report Card, Standardized Test Results or any other information which would be helpful in placing the student correctly.)

Proof of Date of Birth (for Kindergarten Registration Original Birth Certificate must be presented.)

Name, Address and Phone Number of Previous School Attended

For Middle School Students Only: Choice of Foreign Language Study (French or Spanish)
